When you submit a request, we collect the contact details, adult age range, state, selected interests, payment or coverage interest, timing, and contact choices you provide. We keep submission references, the consent wording and version shown to you, dates, internal routing and priority results, communication status, and staff activity records. Our hosting and communications providers may process technical request and delivery information needed to operate their services. This form is for adults 18 and older and is not intended for medical records, diagnoses, medications, insurance member IDs, or payment details.
We use your information to handle your request, organize staff follow-up, communicate through channels you permit, maintain consent and suppression records, and protect the service. Priority scores organize outreach; they do not determine medical necessity, treatment suitability, insurance coverage, or access to care.
Your request comes to MetreCor for internal review. This form does not automatically send your intake to Altro or another clinical or supply partner. Before a proposed handoff, we identify the recipient and explain what will be shared, obtaining any required permission. You can choose to continue to the Altro storefront from the next-step page. The link does not include your contact details or intake answers. The partner's own privacy notices govern information you provide directly to it. MetreCor does not provide medical care.
Hosting, database, and communications providers process information to support MetreCor, including Paubox for email delivery. We do not sell or share mobile numbers or SMS opt-in data with third parties or affiliates for their own marketing or promotional purposes. Messaging consent is used to operate the messaging program, including through service providers supporting delivery.
Acknowledging this notice does not authorize optional follow-up. Email, text, phone, and internal partner-review permissions are separate, optional choices and are not a condition of purchase. You can submit an information request without agreeing to follow-up.
If you select email follow-up and consent, the automated sequence includes a request confirmation and follow-ups approximately 24 and 72 hours after submission. Reply to reach MetreCor. Use the unsubscribe link in any follow-up email to stop future automated emails, or contact admin@metrecor.com. Save the private preference link on your confirmation to withdraw all optional contact permissions. A message already sent cannot be recalled. We may keep a limited suppression record to honor an opt-out.
SMS delivery is not currently active while carrier registration and testing are completed. Selecting text permission records your choice but does not start a working text service. Contact admin@metrecor.com to change a recorded text or phone permission now. See the SMS Program Terms below for the planned service.
The website uses browser storage to remember choices and record local site activity and campaign attribution. The analytics banner controls permission for external analytics and advertising measurement. External analytics and advertising integrations are not currently configured. The intake does not load the website's marketing tracking scripts. You can clear saved browser data through your browser settings; doing so does not delete a submitted request or change its contact permissions.
Contact admin@metrecor.com to request access, correction, or deletion of your information. We may verify your identity before handling a request. Request records, consent evidence, activity history, and suppression records are retained to handle requests, document permissions, and honor contact choices; records are not automatically deleted after a fixed period. We may retain information needed for these purposes or legal obligations, including records held in backups.
We use access controls and other safeguards to protect information, but no system guarantees absolute security. Please do not submit sensitive medical details through this intake. We will post updates to this notice here with a revised effective date.
Updated October 23, 2026. Delivery pending activation. MetreCor LLC's optional text program provides non-clinical request confirmations, next-step reminders, and customer support for adults 18 and older who expressly opt in through this form. It does not provide medical advice or emergency assistance. Please do not send medical records or other sensitive health information by text.
Consent is not a condition of purchase. Message frequency varies: an initial confirmation and up to two automated follow-ups per request, plus responses to your questions. Message and data rates may apply. Messages will come from (954) 710-0441. Delivery depends on your carrier and network; carriers are not liable for delayed or undelivered messages.
Once text service is active, reply STOP to cancel future texts. You may receive one confirmation of your opt-out. Reply HELP for assistance, or email admin@metrecor.com at any time. You can also withdraw contact permission using the private preferences link provided after submitting your request. Only opt in with a number you own or are authorized to use; tell us if it changes.
Your number and messaging consent are used only for this program and its delivery providers, including Twilio for SMS. They are not sold or shared with third parties or affiliates for their marketing or promotional purposes. Read the Privacy Notice above for more information.
